Clinical Analysis of the Normal-pressure Hydrocephalus Syndrome


Abstract
A seriese of 18 patitents with normal-pressure hydrocephalus syndrome from Jan. 1982 to Sept. 1985 at Chonbuk National University Hospital is clinically analyzed. Diagnosis was based on clinical symptoms, the criteria of computerized tomographic (C-T) brain scan and intracranial pressure via lumbar puncture. All patitents recieved the ventriculoperitoneal shunt operation. The following are the results.
1 . N. P. H. syndrome caused by C. N. S. infection occurred in young age, but the idiopathic N. P. H. syndrome developed in old age.
2 . Duration of symptoms in the idiopathic N. P. H. syndrome was longer than others.
3 . Mental change and gait disturbance were predominant among the classical symptoms in the seriese of N. P. H. syndrome.
4 . The findings of the brain C-T revealed moderate ventricular dilatation in all cases, rounding of frontal horn of the lateral ventricle in 13 cases, periventricular low density area in
9 cases, cortical atrophy in 7 cases and obliteration of cerebral sulci in relatively a few cases(6 cases). Five patients among 7 cases with significant cortical atrophy were not improved after the shunting procedure.
5 . For the group as a whole, clinical symptoms were improved in 10 cases, delayed in 4 cases which then discharged with moderate neurological deficit and poor outcome in 4 cases. A case of N. P. H. syndrome followed by craniotomy died of sepsis. Reoperation of V-P shunt was done due to malfunction of shunt tube in one N. P. H. syndrome followed by head injury and he eventually died due to complication of meningitis and sepsis. In two patitents of the idiopathic N. P. H. syndrome, the subdural hygroma was developed after the V-P shunt operation in one case who s",,owed severe cortical atrophy in brain C-T and was suffered from diabetes and pseudobulbar palsy and finally he died due to pneumonia and sepsis. The other one died suddenly of heart failure at 1 week after V-P shunt.
